The basics
- your business name, address, phone number and license number
- the customer’s name and the job address
- an invoice number, the date and a due date
- the fixture with its brand, capacity and serial number
- parts, labor hours and rate, and disposal on separate lines
- the permit number where you pulled one, then the total due
Texas: the first-page block
Texas requires this on the first page of every proposal, invoice or contract for plumbing services, written or electronic, in 12-point type or larger:
- the Responsible Master Plumber’s first and last name and license number
- the words “regulated by the Texas State Board of Plumbing Examiners”
- the Board’s mailing address, phone number and website
The rule counts estimates, service and billing invoices and receipts as proposals, invoices or contracts. The Board’s consumer page gives its phone number as (512) 936-5200.
This template prints notes smaller than 12 point, so on its own it does not meet the Texas rule yet. For Texas plumbing work, add the block in 12-point type to the first page before you send it.

California: license number and vans
California requires your license number on all contracts, subcontracts and bids, and CSLB treats a proposal as advertising, which needs it too. C-36 plumbing contractors also show the business name, address and license number on each side of every commercially registered vehicle, in letters at least 1.5 inches high.
Add your license number as the last line of your address under From, and it prints under your business name on every invoice.

UK: gas work and the Gas Safe Register
A business doing gas work within the scope of the Gas Safety (Installation and Use) Regulations 1998 must be on the Gas Safe Register.
The register’s policy says a business does gas work only under its registered trading name, and that name belongs on its invoices, headed paper, adverts, website and van stickers. Its work records, kept for at least six years, show the engineer’s name, the Gas Safe registration number, the work done with its safety checks, the date and the address.

UK: what to put on a gas job
Put your Gas Safe registration number on every quote and invoice for gas work, with the appliance make and model and the certificate you issued, for example the Building Regulations compliance certificate for a new boiler. Customers check the number.
For work that is not gas, such as a radiator swap, the standard UK invoice rules apply, with VAT only once you are VAT registered.
